[Endeavour, New Journey, New Era, Great Change]

Since the reform of evaluation in 2015, National Medical Products Administration has approved 95 innovative drugs and approved 155 innovative medical devices to be listed.

Five Covid-19 vaccines were approved for conditional marketing, two Covid-19 vaccines were approved for emergency use, and 34 vaccine varieties with five technical routes entered clinical trials.

106 novel coronavirus detection reagents were approved, including 38 nucleic acid detection reagents, 37 antibody detection reagents and 31 antigen detection reagents.

"The quality of registration application materials is not high, and it needs to be supplemented and improved many times during the review process, which seriously affects the efficiency of review and approval; Repeated construction and application of generic drugs, vicious competition in the market, and the quality of some generic drugs is far from the international advanced level; The listing approval time of new drugs urgently needed in clinic is too long, and drug research and development institutions and researchers cannot apply for drug registration, which affects the enthusiasm of drug innovation. "

This is a frank and incisive analysis of drugs and medical devices related to people’s lives and health in a the State Council document seven years ago. This analysis resonates with the confusion of the people: Why are some drugs "slower" to market in China than in foreign countries? Why do some people go overseas to purchase medicines?

The full name of the the State Council document is Opinions on Reforming the Examination and Approval System of Medicines and Medical Devices, which was issued in August 2015, thus kicking off the reform of the examination and approval system of medicines and medical devices in China (hereinafter referred to as the examination and approval reform). Especially since its establishment in 2018, National Medical Products Administration has continuously deepened the reform of evaluation, encouraged the innovation of drugs and medical devices, improved the quality of drugs and medical devices, increased effective supply, and effectively guaranteed people’s health and life safety.

1. The backlog of registration applications has been solved.

The quality of drugs and medical devices is related to everyone’s health and safety, which is the original intention of implementing the review and approval system. However, if the evaluation efficiency is too low, it will lead to the lag of product listing time, which will also affect people’s health and safety. After all, disease waits for no one.

Take drugs as an example. In the past, the backlog of drug registration applications in China was serious, with a maximum of 22,000 applications. Many drugs have to wait several years for approval after they apply for registration. Pharmaceutical companies are helpless and patients are miserable.

How to speed up the review? This is the first problem to be solved in the reform. The drug regulatory authorities, seeing the move, launched a series of powerful measures — —

Full implementation of the implied license system for 60-day expiration of drug clinical trials, which is one-third shorter than the 90-day review time limit before the reform.

We will establish and improve the procedures for accelerating the listing and registration of drugs, optimize the review and approval process, and give priority to the clinical shortage of drugs, anticancer drugs, drugs for children, drugs for rare diseases, drugs for major infectious diseases, vaccines urgently needed for disease prevention and control, and innovative high-end medical devices, thus greatly shortening the review time limit.

Drugs that show efficacy in early clinical trials can be conditionally approved for marketing and used in urgent need patients who can’t wait any longer.

    … … … …

As a drug technical evaluation institution, National Medical Products Administration Drug Evaluation Center has also strengthened the management of evaluation items, refined the evaluation sequence, strengthened the management of time limit, set up special groups, increased reviewers, revised the evaluation points, standardized technical requirements, etc., and the evaluation standards, quality and efficiency of drugs have been greatly improved.

"From the peak in 2015, there was a backlog of more than 20,000 applications. By 2018, more than 90% of them were reviewed within the time limit, and China’s drug review and approval entered the fast lane." The relevant person in charge of the Department of Drug Registration of National Medical Products Administration told the reporter that, for example, in terms of new drugs urgently needed overseas, by the end of 2021, 51 new drugs urgently needed overseas were approved for listing, among which the review of rare diseases drugs was completed within 3 months, and the review of other drugs urgently needed clinically was completed within 6 months, which greatly shortened the time for listing new drugs urgently needed overseas in China.

2. The innovation enthusiasm of the pharmaceutical industry has been stimulated.

The backlog of applications has been solved, and the speed of drug listing has been accelerated. However, in order to meet the people’s drug demand, new drugs need to be put on the market continuously. Encouraging drug innovation is the core of evaluation reform.

In 2015, the Opinions on Reforming the Examination and Approval System of Drugs and Medical Devices raised the standards for drug approval, and changed the new drugs from "drugs that were not marketed in China" to "drugs that were not marketed in China or abroad", that is, "global new". Two years later, the General Offices of the General Office of the Central Committee of the CPC and the State Council issued the Opinions on Deepening the Reform of Review and Approval System and Encouraging the Innovation of Pharmaceutical and Medical Devices, and made special arrangements for the innovation of pharmaceutical and medical devices.

The reporter learned that in order to encourage the research and development of innovative drugs and medical devices, National Medical Products Administration not only gave support in priority evaluation, but also formulated a series of technical guiding principles, such as "Technical Requirements for Application for Pharmaceutical Common Problems in Phase I Clinical Trials of Innovative Chemicals", to guide the application for research and development of innovative products. Reviewers will also take the initiative to communicate with R&D producers in key review links such as pre-clinical, clinical and pre-listing, so as to avoid detours.

"The review reform encourages clinical value-oriented drug innovation, optimizes the review and approval procedures for innovative drugs, and accelerates the review of innovative drugs that are urgently needed in clinic, which greatly stimulates the research and development vitality of pharmaceutical companies." The relevant person in charge of Renfu Pharmaceutical Group Co., Ltd. told reporters that in recent years, a number of new drugs have been approved for listing in this enterprise, and new breakthroughs have been achieved in innovation capability.

Among many reform measures, the system of drug marketing license holders deserves attention. Previously, only pharmaceutical manufacturers could apply for drug registration. In order to register drugs, researchers and research institutions must first invest and build factories themselves. This management system of product registration and production license is not conducive to drug innovation. In 2015, China launched a pilot system of drug marketing license holders in 10 provinces and cities, and drug research and development institutions and researchers can apply for registered drugs. Expert analysis pointed out that this move reduced the capital investment and time cost of drug developers, and new drugs could be listed three to five years in advance. In 2019, the drug marketing license holder system was written into the newly revised drug administration law and was fully implemented nationwide.

According to National Medical Products Administration, in recent years, China has made great progress in the development of innovative drugs. From 2018 to 2021, the number of innovative drugs approved for marketing increased year by year, with 11, 12, 20 and 45, respectively, including Covid-19 vaccines and clinically urgent therapeutic drugs in the fields of tumors, immune system diseases and rare diseases. The pace of medical device innovation is also accelerating. In 2021, 35 innovative medical devices were approved for listing, which is the largest number of innovative products approved in recent years.

3. Consistency evaluation improves the efficacy of generic drugs.

In recent years, many drugs that were originally expensive began to reduce their prices. Careful observation shows that there is a "hero" behind the decline in drug prices — — Generic drugs.

Innovative drugs provide new options for treating diseases and saving lives, but for the vast majority of patients, generic drugs are their life. According to National Medical Products Administration, China’s modern pharmaceutical industry started late, and the production of drugs is mainly imitation. More than 95% of the chemicals that have been approved for marketing are generic drugs.

Generic drugs are imitations of the original drug after the patent protection period expires, and the price is much lower than that of the original drug. In the past, China’s generic drug evaluation and approval standards were not high, and there was a lack of high-quality generic drugs, and the curative effect was very different from that of the original drug.

Improving the quality and efficacy of generic drugs is an important goal of evaluation reform, and the solution is the consistency evaluation of generic drug quality and efficacy — — For generic drugs that have been approved for marketing, the evaluation should be carried out according to the principle of consistency with the quality and efficacy of the original drug. The core is that generic drugs should reach the level consistent with the original drug in quality and efficacy, and can replace the original drug in clinic.

"There are several key issues in carrying out consistency evaluation, one is the selection and determination of reference preparations, the other is the determination of technical standards, and the third is to strictly follow the procedures." According to the person in charge of National Medical Products Administration, the Bureau ensured the smooth development of generic drug conformity evaluation by improving the evaluation system, strictly evaluating standards, strengthening service guidance and optimizing the workflow. At present, 4677 specifications of generic reference preparations have been published, and 2044 specifications and 437 varieties have been applied for through consistency evaluation.

Consistency evaluation puts generic drugs on the right track, and ultimately benefits the people. Since the pilot of centralized drug procurement and use organized by the state was launched nationwide in 2019, a number of domestic generic drugs won the bid. Medical insurance, health insurance and other departments have organized medical institutions to carry out research on 14 representative generic drugs that won the bid, and confirmed that these 14 generic drugs that passed the consistency evaluation are clinically equivalent to the original drugs. Generic drugs with the same efficacy as the original drug are not only cheaper, but also further reduce the price of the original drug under competitive pressure, greatly reducing the burden of patients.

4, emergency review and approval for epidemic prevention and control to provide a strong guarantee.

4 days, which is the time for China to complete the review and approval of the first Covid-19 nucleic acid detection reagent and sequencing reagent in the world.

On January 20, 2020, National Medical Products Administration Instrument Center started the emergency work overnight, and the reviewers stayed up all night to connect with the enterprises one by one, and the review and research and development were carried out simultaneously. On January 26th, the first batch of four in vitro diagnostic reagent products were approved for marketing, which provided an effective detection method for the first line of anti-epidemic.

The outbreak of epidemic situation in COVID-19 challenges the evaluation reform. Xu Jinghe, deputy director of National Medical Products Administration, said at the Boao Forum for Asia in 2021 that the role of drug supervision departments in epidemic prevention and control has changed, from reviewers to instructors, working with enterprises to speed up product listing.

It is understood that National Medical Products Administration made every effort to carry out emergency review and approval during the epidemic. Five Covid-19 vaccines have been approved for conditional marketing, two Covid-19 vaccines have been approved for emergency use, and 34 vaccine varieties from five technical routes have entered clinical trials. Organized the formulation of three technical guidelines for research and development of therapeutic drugs in Covid-19, approved 58 applications for clinical trials of therapeutic drugs in Covid-19, and approved the registration and listing of Qingfei Detoxification Granules, Huashi Detoxification Granules and Xuanfei Detoxification Granules. 106 novel coronavirus detection reagents were approved to effectively meet the needs of epidemic prevention and control.

The epidemic is not over yet, and the emergency review and approval is still going on. As long as people’s health needs it, the evaluation reform will not stop. The drug supervision departments and reviewers have been speeding up on the road of realizing healthy China, so as to run out of acceleration for people’s health.

On-the-spot report of "8.15-8.17" rainstorm and flood in Leshan, Sichuan Province

From August 15th to 17th, Leshan City, Sichuan Province ushered in a new round of heavy rainfall. Coupled with the water from the upper reaches of the Three Rivers, rivers in Leshan City generally flooded. The water levels of Qingyi River and Minjiang River rose sharply, and suffered two rounds of flood peak crossing. The flood peak water level at Xiaogongzui where the three rivers meet was 363.15m, and the flow rate was 23,900m3/s, which was the biggest flood since 1995, causing the dike in Xiaogongzui section of Leshan to overflow by 0.2m to 0.6m..

Compared with the previous round of rainstorm and flood, this round of rainstorm has a wide coverage, large rainfall level and strong hourly rainfall. The flood involves a large number of rivers and the increase is large, making flood control and disaster relief more arduous. In the face of severe tests, Leshan City moved up and down at the news of the flood season, made careful arrangements, made full efforts to meet the challenges, and implemented various flood control and disaster reduction measures in detail to ensure the safety of people’s lives and property.

Rapid mobilization and deployment

"Adhere to the people first, life first, always maintain a wartime state, always be ready, resolutely win the hard battle of flood control and disaster relief, and ensure that the people of the city spend the flood season safely and smoothly." "We must not be afraid of fatigue, continue to fight, and go all out to ensure the safety of people’s lives and property and ensure the safety of the flood season." Leshan Municipal Party Committee and Municipal Government attached great importance to flood control and disaster reduction. Facing this round of heavy rainfall, the main leaders of Leshan Municipal Party Committee and Municipal Government presided over the video dispatching meeting of flood control and disaster reduction in the whole city, deployed the response work, and went to the front line to supervise the flood control and disaster reduction work.

"All levels and departments should make use of the only 30-hour golden gap between the two rounds of heavy rainfall, pay close attention to verification and investigation, pay close attention to danger removal and reinforcement, pay close attention to the Tengku to meet the flood, and pay close attention to the replenishment of flood control materials." According to the trend and forecast of water and rain conditions, Leshan Flood Control and Drought Relief Headquarters timely issued the Emergency Notice on Doing a Good Job in Post-disaster Recovery and Active Response to Storm and Flood in the Later Period, the Emergency Notice on Doing a Good Job in Flood Prevention for Ships on Water, the Emergency Notice on Doing a Good Job in the Prevention of Heavy Rainfall in this Round and the Emergency Notice on Doing a Good Job in Flood Prevention in Leshan Section of Qingyi River, and the current flood of Qingyi River was controlled at 15,000 cubic meters per second (once in 20 years).

Leshan City Flood Control and Drought Relief Headquarters held a video dispatch meeting in time to dynamically judge the trend of rain and water conditions 24 hours a day. On August 15th, it started the city’s level IV flood control emergency response, and on August 16th, it was upgraded to the city’s level III flood control emergency response. Targeted arrangements were made for the deployment of response preparations, and all flood control responsible persons at all levels and those responsible for monitoring, inspection, early warning, transfer, rescue and resettlement at the "last mile" grassroots level arrived at their posts in time.

Prepare in advance

In the face of this round of heavy rainfall, all parts of Leshan maintained a wartime state, mobilized and deployed early, and implemented various flood control and disaster reduction measures in detail to ensure safe flood control.

All parts of Leshan made overall plans to do a good job in the last round of after-rain verification and the new round of before-rain investigation of heavy rainfall, and carried out full coverage investigation on the coastal areas of major rivers, hidden points of ground disasters, reservoirs and dams, construction sites, etc., and timely organized rescue teams to carry out danger removal and danger removal for existing safety hazards. If the rectification cannot be completed in a short period of time, all the people will be responsible for monitoring and duty, and an emergency plan will be formulated. Comb and anchor all ships, and resolutely put an end to ships out of control.

All parts of Leshan once again increased the propaganda of people’s risk-avoidance transfer in key areas such as hidden danger points of ground disasters and dams in the river center, once again organized the masses to be familiar with the transfer route, and actively educated and guided the masses to enhance their awareness of active risk-avoidance. Strictly implement the requirements of "Three Avoidances" and "Three Emergency Evacuations", make every effort to organize people to avoid danger and transfer, and resolutely "turn around and turn around". As of 23: 00 on August 17th, Leshan has transferred 18,341 people. Among them, 5093 people were transferred from the dam in the middle of the river, 3545 people were transferred from the ground disaster, and 9703 people were transferred from the mountain torrents and low-lying areas of rivers. Actively coordinate the municipal traffic police departments to carry out temporary parking in heavy rainfall, and effectively meet the needs of residents’ vehicles to avoid risks.

The flood control safety of industrial and mining enterprises has been strengthened in Leshan, and 29 hazardous chemicals enterprises in the city have implemented flood control measures and temporarily stopped production. There are 35 coal mines in the city, and 13 are in normal production. All of them were temporarily suspended before this round of heavy rainfall. There are 125 non-coal mines and tailings ponds, all of which were temporarily suspended before this round of heavy rainfall.

Hidden dangers of A-level scenic spots and farmhouses were inspected in all parts of Leshan. As of 14: 00 on the 17th, ticket sales in six scenic spots in the city were suspended. Giant buddha suspended the sale of mountain tickets from August 17th, while the daytime tour of the Yangtze River, the "night tour of Sanjiang" and the "night tour of Lingyun Mountain" continued to be suspended. All-out emergency rescue

Affected by the heavy rainfall in the upper reaches, the water level of Minjiang River keeps rising, and the water level exceeds the warning level many times from August 16th to 17th. At 22: 00 on August 16th, the flood peak flow of Wuyou Temple water level station in Minjiang River was 23,900 cubic meters per second, which caused the foundation and slope protection of Laojiangba River embankment in Jindeng Village of Leshan High-tech Zone to be destroyed by 400 meters, seriously endangering the safety of the river embankment, and threatening the safety of 523 households and 1,756 people behind the embankment. The flood control headquarters of Leshan High-tech Zone started the Class III emergency response to flood control in time, and the villages and towns organized villagers to transfer to avoid danger in time, and all 1748 people were transferred except the rescue personnel. More than 500 rescue workers, including armed police, firefighters and militia, immediately entered the actual combat state and started construction 24 hours a day. By 23: 00 on August 17th, a total of 15 rescue machines were put into use, more than 300 tetrahedrons were thrown, more than 1,000 tons of bags were transported and 25,000 woven bags were used for slope protection, and the collapsed dike was robbed and protected, which quickly controlled the danger. Many communities and underground garages in downtown Leshan are flooded, and relevant departments have piled up more than 5,000 sandbags for flood control in many dikes, strengthened defensive duty, set up warning lines and warning signs, and implemented special personnel control to prevent the river from gathering. Dispatch emergency patrol and rescue teams, dispatch large-scale pumping and drainage facilities, power generation facilities and emergency patrol vehicles. For the 27 property communities affected by the disaster, we will fully organize rescue and disaster relief. At the same time, organize personnel to conduct 24-hour on-site duty and inspection on important infrastructure such as bridges, water supply and dikes along the river.

In order to ensure the safety of transportation, Leshan City has strengthened the safety supervision of "two passengers and one danger" enterprises, buses connecting every village and ferries crossing the ferry, and strengthened the investigation of accident-prone sections, dangerous sections, sections facing the water and cliffs, and sections facing the ridge. During this round of rainstorm and flood, the city’s roads collapsed by 3,000 cubic meters, and the broken road at K1802+650 on the G348 line of Qiqigou, San ‘e Village, Shawan District, National Highway 348 is being repaired. Seven broken roads in rural areas have been rushed through, 27 lines have been suspended, 2 vehicles have been stopped, 142 rural passenger vehicles have been stopped, and 314 ships of various types have been fixed.

Jiangsu People’s Medical Institute introduced three academicians a week, and many 3A hospitals in Nanjing built academician workstations.

At present, the battle for talents in various places is in full swing, and top talents are even more "fragrant". Hospitals are no exception. Last week, the provincial people’s doctor introduced three academicians a week, which attracted the industry’s surprise. During the interview, the reporter learned that the introduction of flexible talents has been surging in Nanjing. Many 3A hospitals have introduced academician workstations, and the First Hospital of Nanjing has invited top national cardiac surgeons to perform 150 operations in three months …

Save people’s doctors and introduce three academicians a week

From June 23rd to June 27th, the third batch of flexible high-end talents from jiangsu province hospital signed a contract in this hospital. Academician Ning Guang of Ruijin Hospital affiliated to Shanghai Jiaotong University School of Medicine, Academician Yu Jinming of Shandong Cancer Hospital, Academician Ge Junbo of Zhongshan Hospital affiliated to Fudan University, Professor Qin Shukui of Bayi Hospital of the People’s Liberation Army, and Professor Li Jin of Oriental Hospital affiliated to Tongji University signed an employment agreement with the hospital, officially becoming a special medical expert in the hospital and signing a contract to become distinguished professor of South Medical University; And the "Academician Workstation" led by two academicians, Yu Jinming and Ge Junbo, also settled in the hospital. So far, the hospital has introduced 12 domestic and foreign high-end medical talents in three batches to settle down flexibly in the province, which has become a new attempt for the hospital to introduce top talents.

It is understood that in the past two years, The hospital has signed talent introduction agreements with many experts, such as Professor Jiang Zefei from the 307th Hospital of the Chinese People’s Liberation Army, Professor Ji Jiafu from Peking University Cancer Hospital, Professor Zhou Pinghong from Zhongshan Hospital affiliated to Fudan University, Professor Huo Yong from the First Hospital of Peking University, Professor Xuanyuan Qingshi, Director of HiPep Institute in Japan, Professor Liang Hanguang, Director of Surgery of Seoul National University, and Academician Li Zhaoshen, Director of Gastroenterology Department of Changhai Hospital affiliated to the Second Military Medical University. Together with the five distinguished professor introduced this time, 12 top talents have settled in the hospital, involving oncology.

"First-class centers call for first-class talents, and first-class talents look forward to a first-class platform. I believe that signing experts can also play their greater value in jiangsu province hospital, a high-level platform." Dean Tang Jinhai of jiangsu province hospital said that the hospital attaches great importance to internationalization, intelligence and talent superiority, and the introduction of flexible talents will quickly help the hospital master advanced medical technology, enhance its service capacity and benefit the people.

Three academicians of Gulou Hospital have been quietly "working" for two years.

It is understood that in the introduction of academicians, Nanjing Gulou Hospital is also a big deal. As early as two years ago, Gulou Hospital took the lead in introducing the academician workstation model, and now three academicians have quietly "worked" for two years.

Dai Yutian, director of the Science and Technology Department of Nanjing Gulou Hospital, introduced that in 2016, the hospital signed contracts with academicians Ning Guang, Liu Zhihong and Qiu Guixing respectively to establish academician workstations. Among them, Academician Ning Guang, a famous endocrinologist from Ruijin Hospital affiliated to Shanghai Jiaotong University School of Medicine, Academician Liu Zhihong from Nanjing General Hospital in urology and Academician Qiu Guixing from Peking Union Medical College Hospital in orthopedics were introduced. Several experts come to Gulou Hospital regularly or send people to listen to the department development and clinical research reports, give clinical and academic guidance, train relevant technical innovation talents, and put forward suggestions and opinions, so as to "find the difference by benchmarking". Dai Yutian introduced that the establishment of the academician workstation has trained talents, improved the clinical and scientific research level of the hospital, and enabled the people to enjoy the quality services of top medical experts at their doorstep.

Dai Yutian introduced that under the leadership of the Nanjing Municipal Health Planning Commission, in the next step, Gulou Hospital will continue to build new academician workstations to further enhance the hospital’s talent strength and clinical research level and better serve patients.

Dr. Yang from the First Hospital performed 150 operations in three months.

Also on May 18th this year, a foreign doctor appeared at the awarding ceremony of Jinling Friendship Award of Nanjing Municipal People’s Government in 2018. Professor Mike Carmack, the chief physician of Cardiovascular Surgery in Nanjing First Hospital (American, MD), was awarded the "Jinling Friendship Award" by Nanjing Mayor Lan Shaomin, becoming one of the ten foreign experts who won this honor in the city.

It is understood that Professor Mike Carmack is a famous expert in cardiovascular surgery in the United States. He has been engaged in clinical work of cardiothoracic surgery in the United States for a long time since 1984. He has a very comprehensive cardiac surgery technology, especially in severe heart bypass surgery, complex heart valve surgery, reoperation, aortic surgery (aneurysm and aortic dissection) and heart transplantation. He is "the fifth doctor in the world who has performed total artificial heart implantation" and enjoys a high reputation in the world. As an expert selected by Jiangsu’s "Hundred Talents Program", Professor Mike Camack officially joined the Cardiovascular Surgery Team of the First Hospital on February 22 this year, and signed a three-year employment agreement to become a full-time foreign doctor in the First Hospital.

Qin Wei, Ph.D., Cardiovascular Surgery, First Hospital, introduced that Professor Carmack has been fully involved in cardiovascular surgery, undertaking tasks such as expert outpatient service (half a day each week in the headquarters and Hexi Campus), ward rounds, surgical treatment and personnel training. Now, Professor Carmack takes part in the early handover of departments every day, and the handover of departments adopts English communication, which greatly improves the overall English level of departments. Professor Carmack takes part in preoperative discussion every day, making operation plans, watching films with young doctors, participating in clinical decision-making, guiding doctors to operate, etc. In particular, he advocated the concept of rapid postoperative recovery, so that patients can leave the ventilator as soon as possible after surgery, remove the drainage tube and get out of bed as soon as possible, which greatly shortened the hospitalization time.

As of May 24th, Professor Carmack has participated in 150 operations, and 91 cases of difficult operations have been performed by him, including two cases of heart transplantation, aortic valve replacement combined with ascending aorta replacement combined with heart bypass, and the second operation for patients after heart surgery. In the past three months, there have been three operations every day on average, and no one has died. Such a low mortality rate is also very rare in the United States.

A number of 3A hospitals introduced academician workstations.

During the interview, the reporter found that in addition to the Provincial People’s Hospital and the Drum Tower Hospital, a number of top three hospitals in Nanjing constantly "borrowed" from the top technical fields in the country and the world, and invited academicians and Nobel Prize winners to set up workstations.

Nanjing Zhongda Hospital also cooperated with Sun Yinghao, an academician of China Academy of Engineering and a national academic leader in urology, to set up an "academician workstation". It is understood that in addition to setting up academician workstations in urology, Zhongda Hospital will also strengthen its strength in surgical disciplines such as cardiothoracic surgery and general surgery by setting up academician workstations. According to the strategic plan of Nanjing No.1 Hospital, the hospital will introduce 8 academicians in recent years, involving oncology, cardiovascular, nuclear medicine and other departments. At present, five academicians, including Han Yaling, Chen Zhinan, Zhan Qimin, Hu Shengshou and Zhong Shizhen, are stationed to guide the work.

Graded diagnosis and treatment, big hospitals urgently need top talents.

An industry expert said that the "graded diagnosis and treatment" promoted by this round of medical reform has made large and small hospitals more clear about their respective positioning. "Minor illnesses are in the community, and serious illnesses go to the hospital." The top three hospitals are no longer "grasping the eyebrows and beards", but taking on more and more difficult diseases. The requirements for technology are getting higher and higher, so the demand for top medical talents is more urgent. However, the dilemma of long training period and difficult mobility of top doctors also exists objectively. How to win more top doctors for hospitals? Academician workstation has become a feasible mode, which has been paid more and more attention by hospitals and departments.

By "marriage" with academicians of the two academies, on the one hand, the hospital flexibly introduces high-level talents, participates in the research of international frontier scientific research projects, and improves the ability to treat difficult and complicated diseases; On the other hand, it is undoubtedly a win-win situation for the hospital to learn from academicians and experts and train a group of academic leaders and key talents, so as to rapidly improve the scientific research and clinical strength of the hospital, further strengthen its own strength and benefit more patients.

Guangdong entered the local epidemic season of dengue fever, and experts warned against dengue fever when traveling abroad.

Xinhua News Agency, Guangzhou, August 11th (Reporter Xiao Sisi) The Guangdong Provincial Health and Wellness Commission issued a health reminder on the prevention and control of dengue fever. At present, Guangdong has entered the local epidemic season of dengue fever, and the imported cases of overseas infection and local cases are gradually increasing. The Guangdong Provincial Health and Wellness Commission reminded that dengue fever spreads rapidly, and there is no specific treatment drug. Severe cases can cause severe bleeding, shock and even death. Dengue fever can be prevented and controlled, and the key lies in prevention.

According to the relevant person in charge of the Guangdong Provincial Health and Wellness Committee, dengue fever in Guangdong Province is mainly transmitted through the bite of Aedes albopictus (Aedes albopictus), eliminating mosquito breeding sites (indoor and external water accumulation in various homes and workplaces), killing adult mosquitoes and preventing mosquito bites by individuals are key measures for dengue prevention and control.

In addition, travel to prevent mosquitoes and keep up with the awareness of disease prevention. Travellers who are going to areas with dengue fever should take mosquito prevention measures, such as wearing light-colored long-sleeved underwear and using mosquito repellent containing deet. If you have fever, skin flushing, muscle and bone pain and other symptoms within 15 days after traveling in dengue endemic areas, you should seek medical treatment as soon as possible, and explain the travel history and health status of your colleagues to your doctor. Patients who have been diagnosed with dengue fever in medical institutions should actively cooperate with the hospital to do isolation treatment and actively prevent the occurrence of serious diseases.

China’s ration self-sufficiency rate exceeds 100%, and there is no gap between grain supply and demand.

This year, China’s grain production has withstood many tests, such as epidemic, flood, typhoon, snowfall and meadow moth, and it is still expected to get another bumper harvest. The Economic Daily reporter learned from relevant parties that China’s total grain output is expected to exceed 650 million tons this year, and the grain supply is sufficient, so China has the ability to secure its rice bowl.

There is no gap between grain supply and demand.

A few days ago, a research institute released a report saying that by 2025, there may be a gap between grain production and demand of about 130 million tons in China, of which the gap between grain production and demand is about 25 million tons. Some people think that China will face food shortage.

In fact, there is a gap between grain production and demand, not a gap between grain supply and demand. At present, China’s grain stocks are sufficient, and the stored rice and wheat can feed the national consumers for more than one year. The self-sufficiency rate of corn is over 95%, and the grain supply can fully meet the market demand.

Xi Yinsheng, a researcher at the Rural Economic Research Center of the Ministry of Agriculture and Rural Affairs, told reporters that the gap between grain production and demand and the gap between supply and demand are two different concepts. The gap between production and demand refers to the shortage of annual output compared with demand, and the shortage is the gap between production and demand. The gap between supply and demand refers to the situation that the supply is not enough to meet the consumption demand. In addition to the output, the grain supply also includes the effective supply and import of the annual inventory. At present, there is no gap between supply and demand in China’s grain.

The gap between grain production and demand in China did not begin this year, mainly because of the limited soybean production capacity in China. The annual demand for soybeans in China is about 110 million tons, and more than 90% of soybeans need to be imported. 80% of imported soybeans are processed into feed and 20% into edible oil, which has little effect on the safety of rations. Cereals mainly include wheat, rice, corn, barley, sorghum, etc., among which wheat, rice and corn are also called the three staple foods, and wheat and rice are collectively called the two major rations. According to the white paper "Food Security in China" issued by China in 2019, the self-sufficiency rate of wheat and rice in China exceeds 100%, and the self-sufficiency rate of grain exceeds 95%.

At present, China’s grain production and demand are basically balanced, and there is no obvious gap between production and demand, but there are structural contradictions. Xi Yinsheng believes that the gap between grain production and demand in China is 25 million tons, mainly referring to the gap between corn production and demand. There is a big gap between corn production and demand, not because China’s production capacity is not enough, but because it is the initiative and inevitable requirement for China to promote the structural reform of agricultural supply side in recent years. At present, the widening gap between corn production and demand is only a phased problem of the adjustment of supply and demand, not a long-term problem. In the future, under the influence of market mechanism, China’s corn production is likely to resume growth, and the gap between production and demand is expected to gradually narrow and achieve a basic balance.

Although there is a gap between corn production and demand in China, there is no gap between supply and demand. In terms of output, although this year has been affected by the typhoon, it is still higher than last year. From the sales of stored corn, this year, the auction turnover of temporary storage corn in China exceeded 56 million tons, some of which have not yet been delivered out of the warehouse, and some of them have not been consumed after delivery, but have been transferred from state inventory to private inventory. According to the delivery schedule, it is estimated that more than 20 million tons of temporary storage auction corn will be carried forward to next year’s consumption. From the perspective of imports, from January to July this year, 4.57 million tons of corn was imported, up 30.7% year-on-year, and the market supply was completely no problem.

Superposition factors lead to food price fluctuation.

Data from the agricultural sector show that the output of summer grain and early rice increased this year, reaching 340.2 billion Jin, an increase of 4.48 billion Jin over the previous year, especially early rice, which reversed the downward trend for seven consecutive years. The harvest of autumn grain is drawing to a close, and it is estimated that the annual grain output will hit a record high again, and it will remain above 1.3 trillion Jin for six consecutive years, firmly holding the rice bowl in its own hands.

Xi Yinsheng believes that the epidemic situation has basically no obvious impact on grain production. Although floods have affected rice production in some areas, from the overall situation of the country, the years with sufficient precipitation are generally beneficial to grain production, and the probability of high yield is high. Three consecutive typhoons in Northeast China have caused maize lodging in some parts of Northeast China. However, due to the generally good growth of maize in the early stage, most of the yield of maize in Northeast China has basically formed when the typhoon occurred, and the actual impact is limited.

Adequate grain stocks are the "anchor needle" to stabilize the market. China’s grain storage consists of three categories: government storage, policy storage and enterprise commodity storage. Government reserves, including central grain reserves and local grain reserves, are "ballast stones" to ensure food security. Policy inventory is the inventory formed by the state’s policies of minimum purchase price and temporary storage. The inventory quantity is considerable and it is auctioned in the market all the year round. According to the data of the National Grain Trading Center, as of October 29th, this year, a total of 203 national policy grain auctions were organized, with a total auction volume of 96.956 million tons, effectively meeting the market demand. Enterprise commodity inventory refers to the self-owned inventory that enterprises need to establish for business turnover. At present, there are more than 40,000 enterprises in the system, and the inventory quantity is not small. "In recent years, the operational risk awareness of China’s grain enterprises has been further enhanced, and the willingness to build more stocks and master more grain sources has been significantly improved. At present, the commodity stocks of enterprises that have entered the system have increased by more than 20% year-on-year." Qin Yuyun, director of the Grain Reserve Department of the State Grain and Material Reserve Bureau, said.

The food supply is guaranteed, but the prices of rice, wheat and corn have all increased to varying degrees this year, especially since October, the price of corn in Northeast China has increased greatly. Xi Yinsheng believes that the increase in grain prices this year is the result of the superposition of many factors. First of all, the global spread of the COVID-19 epidemic has aroused the international community’s concern about food security. Some countries have taken measures such as restricting exports, which has pushed up international food prices and transmitted them to China, thus affecting the trend of food prices. Secondly, the typhoon caused corn lodging in some parts of Northeast China, which led to a significant increase in corn harvest costs and pushed up corn prices. At the same time, the corn harvest and listing were delayed, and farmers’ reluctance to sell further strengthened the expectation of corn price increase. Thirdly, the boosting effect of market speculation on grain price increase is very obvious. Since the beginning of this year, hot money speculation on corn and other price increases are expected to be obvious, and frequent speculation on topics such as weather and epidemic situation has contributed to the increase in food prices.

From food security to food security

Food security has certain levels, which can be generally divided into three levels: ration security, grain security and food security. At present, China has achieved the goal of "basic self-sufficiency in grain and absolute safety in rations", and the food security situation is at its best in history.

Since the 18th National Congress of the Communist Party of China, China has established a national food security strategy of "focusing on ourselves, basing itself on the domestic market, ensuring the production capacity, moderately importing and supporting by science and technology", and it is clear that we should rely on our own strength to concentrate the main domestic resources to ensure the basic self-sufficiency of cereals and the absolute safety of rations. Guoqiang Cheng, a member of the National Food Security Strategy Expert Advisory Committee and school of economics and management of Tongji University, believes that after years of unremitting efforts, China has established a "trinity" food security guarantee mechanism, that is, a domestic food production system with the core of ensuring the absolute safety of rations; A grain reserve system focusing on responding to all kinds of emergencies and maintaining market stability; The global agricultural product supply chain, which aims at making overall use of the international grain market and resources, supports and cooperates with each other. Among them, the domestic grain production system and reserve system embody the food security policy of "taking myself as the main factor and basing ourselves on the domestic market" and "grain is basically self-sufficient and rations are absolutely safe", while the global agricultural product supply chain is the policy design and path selection of "moderate import". By moderately importing non-staple agricultural products such as soybeans and meat, the pressure of insufficient domestic water and soil resources is alleviated and agricultural resources are concentrated to ensure rations production.

Food security is actually food security. According to the definition of FAO in 1983, the goal of food security is to "ensure that all people can buy and afford the basic food they need at any time". Nowadays, China’s food security has changed from the pursuit of "eating well" to "eating well", and the perspective of examining food security has shifted from food security to food security, and on the basis of ensuring "basic self-sufficiency of grain and absolute safety of rations", food sources have been continuously expanded. It is noteworthy that in recent years, while the rigid demand for food in China is increasing, the food consumption structure is also undergoing important changes, the consumption of rations is gradually decreasing, and the consumption of miscellaneous grains, vegetables, fruits, aquatic products and livestock products is increasing. Between "one drop and one increase", it reflects the trend of more diversified food demand of Chinese consumers.

"Under the new situation of diversified food consumption, we must establish a big food safety concept and attach importance to ‘ Rice bag ’ We should also pay attention to safety ‘ Shopping basket ’ Safety, build a diversified whole food industry system, meet people’s increasingly diverse food consumption needs, and protect residents’ food and nutrition needs at a higher level. " Xi Yinsheng said.

Birds fly, fish jump, and Chongming is fresh (the river flows-a report from the Yangtze River Economic Belt)

Chongming, the world’s largest estuarine alluvial island, acts as an ecological barrier for Shanghai, a megacity, where the Yangtze River enters the sea.

Beside Qingcaosha Reservoir in Changxing Island, one of Chongming Island, more than 700 mu of ecological forest separates the reservoir from the living and production area. All kinds of fry in the reservoir area thrive and the biodiversity is effectively protected. "This is not a stagnant pool, but a living ecosystem." Zhu Yiping, director of the Water Quality Center of Chongming Shanghai Chengtou Raw Water Company, said that this water area, which is equivalent to 10 West Lake in Hangzhou, provides clean drinking water sources for more than half of Shanghai’s population.

Chongming Dongtan is one of the eight major migration routes of birds in the world, with more than 1 million migratory birds inhabiting or transiting here every year. In the past, due to the destruction of wetland ecosystem, the species and number of birds decreased significantly. Since 2013, the ecological restoration project of Chongming Dongtan has been implemented in Shanghai to optimize the bird habitat within 24.2 square kilometers of the protected area. Reed, sea bream, etc. were gradually restored, and the degraded wetland ecosystem was improved. "The number of migratory birds migrating and transiting in 2017 was twice that of 2016." Observing this change, Yuan Lin, an associate researcher at the State Key Laboratory of Estuarine and Coastal Studies of East China Normal University, was pleasantly surprised.

East of Dongtan, the Chinese sturgeon, which is listed as a "critically endangered" protected species, is being carefully cared for. In the aquatic biological protection base here, Chinese sturgeons, large and small, have been rehabilitated or are multiplying. As the director of the management department of the Chinese sturgeon nature reserve in the Yangtze River Estuary of Shanghai, Liu Jian ‘ai said: "The Yangtze River ecology is good or not, not people, but fish." Up to now, the reserve has carried out more than 20 times of proliferation and release, and hundreds of thousands of rare aquatic organisms such as Chinese sturgeon and mullet of various specifications have been released.

It is necessary to let birds fly and fish swim, but also to make the lives of ordinary people rich and beautiful. More than 10 years ago, Shanghai made a decision to build Chongming Ecological Island, and rolled out a three-year action plan. In February 2017, Shanghai formulated and promulgated the "Thirteenth Five-Year Plan for the Development of Chongming World-class Ecological Island". The ecological construction station is higher, the principle of ecological island establishment is firmer, the concept of not engaging in large-scale development is more determined, and the direction of green development is more distinct.

Aiming at Chongming, a world-class ecological island, we will strive to build a new benchmark and demonstration area where people live in harmony with nature and give priority to ecological green development, explore environmental protection models and industrial development models that adapt to our own characteristics, and seize the key to sustainable ecological improvement and industrial development.

Over the years, Huang Shiying, a 68-year-old villager in Xincun Township, has seen the environmental changes. "In the past, the river was muddy, with a lot of weeds and garbage." Unexpectedly, there is Chongming’s first fixed-point delivery point for rural domestic waste at regular intervals near home. Through the mode of "household separation, household investment, rural transportation and rural disposal", the classified garbage is treated in a unified way.

In Sanxing Town, the purification tank behind the house of villager Huang Hanbin absorbed the domestic sewage of several villagers nearby. This is a low-cost sewage treatment equipment introduced by the town according to local conditions, which makes "the river water better and the flies and mosquitoes less". Once considered to be the most difficult to implement garbage sorting, Chongming District basically completed it in one year, and now it has achieved full coverage of garbage sorting.

The way of ecological priority and comprehensive improvement, Chongming is getting wider and wider. The development of eco-industry focusing on emerging industries such as urban modern green agriculture, eco-leisure tourism and marine equipment manufacturing is also booming.

Chongming has selected 10,000 mu of land in the whole region to carry out the production of "two-chemical-free" rice without chemical fertilizers and chemical pesticides. The first batch of new rice will be listed soon this year, and farmers are expected to increase their income by 20 million yuan. "Although the yield per mu will be much lower, we will build sales channels and win by quality." Shi Yuping, deputy general manager of Beihu Agricultural Development Co., Ltd. is very confident. In the future, Cuiguan pears, oranges and vegetables, which are "two-for-nothing", will also become the superior products of Chongming’s green agriculture.

In Changxing Town, east of Qingcaosha Reservoir, the marine equipment industry has begun to take shape. Changxing Town takes marine equipment and supporting, intelligent manufacturing industry as the development focus, and promotes intelligent production lines and green shipbuilding technology. Chongming has set the highest green development threshold in the city. On the one hand, it optimizes the negative list of industrial access, on the other hand, it guides the island’s industries to concentrate in the park, develops producer services that meet the requirements of ecological environment protection, and builds a demonstration area of green economy.

Good air can also be eaten as a meal. Today, Chongming Island attracts more and more tourists, with 3.25 million visitors in the first half of this year alone. With Chongming District being qualified to host the 10th China Flower Expo in 2021, this green ecological island near the river and the sea will be transformed into a colorful "flower island on the sea", showing the beauty of ecological harmony and the charm of green development, adding luster to Shanghai.

At 0: 00 on January 1, 2020, these two toll stations will charge at the same time.

On May 16th, 2019, the State Council issued the "Implementation Plan for Deepening the Reform of Toll System and Cancelling Provincial Toll Stations of Expressway", and deployed the working goal of "Strive to basically cancel provincial toll stations of expressway before the end of this year". Among them, there are two toll stations in Nanjing Guan Expressway, namely, Xingdian Toll Station of G40 Hushan Expressway (referred to as Xingdian Toll Station) and Ningma Toll Station of G4211 Ningwu Expressway (referred to as Ningma Toll Station).

The provincial boundary toll stations of Ninghe Expressway include the original Xingdian toll station and Wuzhuang toll station. Among them, Xingdian Toll Station is located in Jiangsu, 5.4 kilometers away from the Jiangsu-Anhui provincial boundary, and is responsible for collecting Jiangsu tolls; Wuzhuang Toll Station is located in Anhui Province, 6.2 kilometers away from the Jiangsu-Anhui provincial boundary, and merged into the networked expressway section of Anhui Province. In order to realize the rapid passage of vehicles, the former Jiexingdian toll station of Ninghe Expressway in Jiangsu Province will be officially demolished with the joint coordination of Jiangsu and Anhui provinces and the approval of the Ministry of Transport. From 0: 00 on January 1, 2020, the toll station will be charged in parallel with the Wuzhuang toll station of Ninghe Expressway in Anhui Province.

After the plan was clear, under the guidance of the provincial highway center, Nanjing overcame the difficulties of tight construction period and high construction risk, carefully organized and made overall arrangements, and carried out the demolition work in an orderly manner in strict accordance with the demolition plan approved by experts. In only 9 days, the facilities such as the safety island, the toll booth and the toll shed above the main line in the six lanes of the main line in the station area were demolished, and the road surface restoration, guardrail installation, marking and marking were completed. On December 20, the goal of straightening the main line of the highway was successfully completed. On January 1, 2020, after the parallel station charging is implemented in Wuzhuang Toll Station of Ninghe Expressway in Anhui Province, the original Xingdian Toll Station area will realize two-way four-lane traffic.

The ETC reconstruction of Ningma Toll Station of Ningwu Expressway has been completed.

Ningma Toll Station of Ningwu Expressway is located in Nanjing, Jiangsu Province, 330 meters away from the Jiangsu-Anhui provincial boundary, and is co-located with Anhui (Anhui implements closed network toll collection). Nanjing is responsible for the charge management in the direction of entering Anhui, and issues a pass card for the entrance of Anhui road network. According to the Overall Technical Plan for Cancelling Provincial Toll Stations of Expressway issued by the Ministry of Transport, the Letter on Cancelling Provincial Toll Stations of Expressway in Jiangsu and Anhui Provinces and the spirit of the meeting on related matters of provincial toll stations in Jiangsu and Anhui Provinces, Ningma Toll Station is temporarily retained as a port station for controlling the entrance and exit of toll sections of closed-end networked expressway in Anhui Province, and the transformation of the station is jointly implemented by Jiangsu and Anhui. At present, the reconstruction of ETC lane of Ningma toll station has been completed.
